WELCOME TO MIND FORWARD BRAIN INJURY SERVICES

Mind Forward Brain Injury Services is a not-for-profit charitable organization, completely funded by Ontario Health to provide community based rehabilitation and life-long support for adults whose lives have been touched by acquired brain injury.

Since incorporating in 1992, Mind Forward has been committed to offering services to all individuals touched by brain injury (including clients, family members and caregivers) based on an assessment of the individual’s needs and through a team approach to service planning and goal-setting. The client and significant others are welcomed as members of the program planning process, along with our staff and other experts.

Your donation to Mind Forward Brain Injury Services will help us continue to support our clients and families whose lives have been changed by an acquired brain injury. Mind Forward Brain Injury Services does not apply an administrative fee or percentage to the processing of donations. Every dollar donated goes directly toward Agency programming.
We appreciate many forms of donations and accept monetary, merchandise, services and tribute donations. A charitable tax receipt will be sent to the donor through CanadaHelps.org or by Mind Forward Brain Injury Services.  DONATE NOW…

Equity, Diversity and Inclusion Committee and news

In November 2020, the Mind Forward Equity, Diversity and Inclusion Committee met for the first time to begin a journey like no other. The committee is a dedicated group of individuals with commitment to change. COMMUNITY PROGRAMS

COMMUNITY PROGRAMS

Mind Forward Brain Injury Services offers a variety of community based programs across the MH and CW LHINs.
Our Seniors program includes provision of specialized staffing resources embedded within long-term care facilities to maintain placement for individuals whose behaviors may place them at risk.
The Supported Independent Living (SIL) program supports individuals who do not require the same level of support offered in ABI specialized assisted living or LTC; however are unable to live independently without putting their safety at risk. Our team provides ongoing in-home and community support which includes functional skills assessment and training.
We also offer three adult Day Programs in Mississauga, Orangeville and Oakville where clients have the opportunity to participate in a variety of groups and special events ranging from social and recreational activities, exercise and fitness, functional skill-building groups and daily drop-in hours.

ASSISTED LIVING PROGRAMS

Mind Forward Brain Injury Services provides assisted living beds with 24 hour/ 7 days a week staff support in Oakville, Mississauga and Brampton. Mind Forward Brain Injury Services supports five residential environments. These sites are designed to support a range of individuals, from highly structured living environments to long-term supported living. All residential placements are based on assessed need and focus on maintaining independent living skills and social recreational engagement in the residence and in the community.

Mind Forward Clinical Services

CLINICAL SERVICES

Mind Forward Brain Injury Services offers a broad range of Clinical Services, including Psychological assessment and treatment (with access to a Psychologist on staff and a second consulting Psychologist), Neuro-Psychiatry consultation for medication management, access to a Registered Social Worker, behavioural consultation, family/caregiver counselling and support, and client counselling.

Mind Forward Brain Injury Services is also an approved Ontario Telehealth Network (OTN) hub to increase access to Clinical Services and community agencies.

UNMASKING Brain Injury

This month represents Brain Injury Awareness. Every year, Mind Forward celebrates survivors who are encouraged to share their life stories through art, discussion, and/or social media. With the support of Headwaters Acquired Brain Injury Group (HABI), and Mind Forward, these masks were submitted to Ontario Brain Injury Association (OBIA). Read more…
